How and why do polyatomic ions form?

Ammonium, for example, forms from nitrogen and four hydrogen atoms. Isn't there a stable NH3 compound already? Why does NH4 form? I don't understand this. I think nitrogen loses an electron in order to form NH4. Where does this electron go? What about nitrate? How does that form? I don't understand why polyatomic ions form.
